We offer skilled nursing services, home health aide services, physical therapy, and limited speech therapy based off geographical location.
We work closely with the patient, their physician, and caregivers to provide a personalized plan of care tailored to the patient’s specific home health needs. Our Ivy Creek Home Health of Lake Martin team consists of licensed healthcare professionals who offer in-home oriented treatment alternatives to patients who have recently been released from the hospital, skilled nursing facility, and/or rehabilitation facility, have experienced an issue with a current medical condition, or have been newly diagnosed and require disease management education.
Our nurses excel in providing and educating our patients and caregivers on wound care, IV drug therapy, foley catheter care and changes, patient safety, as well as providing education on disease processes and medication. Our therapy staff provides skilled in-home therapy while developing home exercise programs to promote improved functioning and mobility for our patients.

Ivy Creek Home Health of Lake Martin has been a Medicare-accredited Home Health Care Agency since July 29, 1993. Our parent agency is located at 729 East South Street, Dadeville, AL. We have a branch office located at 854 Airport Drive, Alexander City, Alabama. We are a state-licensed home health agency in Alabama.
Our Home Health teams consist of healthcare professionals offering home oriented alternatives to patients that are homebound and suffering from cardiovascular disease or diabetes, have a need for rehabilitation from surgery or have experienced a recent change in other health conditions requiring skilled interventions in the home. Some of these interventions and services may include skilled nursing education, wound care, physical therapy or speech therapy based on the patients’ needs and geographical location.
To qualify for Home Health a patient must have an attending physician to order home services, the need for part-time or intermittent skilled services such as nursing, speech therapy or physical therapy and you’re homebound.
