Duties

  • Provide comprehensive nursing assessments and develop individualized care plans tailored to each patient’s needs.
  • Administer medications, including injections and IV infusions, following strict protocols and safety standards.
  • Monitor vital signs, perform physical examinations, and evaluate patient responses to treatments.
  • Educate patients and their families on disease management, medication use, and health maintenance strategies.
  • Coordinate with interdisciplinary teams using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to ensure seamless communication.
  • Manage complex cases such as dementia, Alzheimer’s, memory care, hospice needs, and acute pain management.
  • Conduct triage assessments for urgent issues, ensuring timely intervention and appropriate escalation of care.

Qualifications

  • Valid RN or LPN license in good standing with relevant state licensing boards.
  • Experience with Epic or similar EMR/EHR systems preferred; familiarity with Cerner or Athenahealth is a plus.
  • Critical care experience in settings such as ICU, emergency room, or trauma centers highly desirable.
  • Knowledge of ICD-9/10 coding for accurate documentation and billing processes.
  • Proven ability to work with individuals with developmental disabilities, dementia, or in assisted living environments.
  • Strong understanding of medical terminology, physiology, and diagnostic evaluation techniques.
