Lake Martin Community Hospital is looking to hire Full Time, Part Time or PRN LPNs for day/night shift on our Med/Surg floor.

7a-7p or 7p-7a.

Pay is to be determined based off experience.

Qualifications: This position requires a licensed practical nurse with a current Alabama state license as a licensed practical nurse.

ESSENTIAL JOB DUTIES AND RESPONSIBLITIES:

  • Collects and documents physical, social, and psychological data systematically from all available sources, i.e., family, patient, records, etc.
  • In collaboration with the Registered Nurse, nursing staff and physicians, gives direct care to all patients assigned and is responsible for keeping the Registered Nurse informed about patient’s condition and needs.
  • Makes hourly walking rounds to assess and re-assess patients and evaluate the effectiveness of nursing care plan.
  • Performs a wide variety of treatments and procedures, such as medication administration, vital signs, specimen collection, accu-checks, dressing changes, wound irrigations, suture removal, Foley catheterizations, diabetic teaching, isolation procedures, compresses, colostomy care, decubitus care, enemas, patient safety, etc.
  • Sets up and assists physician with routine physical examinations, including pelvic examinations, etc. after explaining the procedure(s) to the patient and attaining appropriate consent from with signature(s) if indicated.
  • Promptly notifies physician of situations requiring immediate intervention and accurately implements orders as received.
  • Bathes, feeds, gives back rubs, assists patients with ambulation, and provides routine services for patient comfort, well-being and safety.
  • After completing IV course, initiates IV’s, administers routine admixtures and IV fluids according to hospital policy.
  • Observes patients for physical and emotional reactions to medications and treatments, reporting irregularities to the RN.
  • Documents any omitted medications and explanation for omission, notifies physician of omission and/or any response to PRN medications.
  • Charts patient’s symptoms, reactions, progress, treatments performed, nursing care and any other pertinent information on the patient’s medical record.
  • Assists with transcription of physician orders and assumes responsibility that orders are carried out.
  • Applies telemetry as ordered and is familiar with recognition of changes and lethal arrhythmias.
  • Notifies Home Health/Hospice immediately of any consults ordered by the physician, so that prompt visits may begin.
  • Assists in other departments as needed.
  • Active participant in training and evaluation of nursing assistants and ward clerks.
  • Attends and participates in patient care rounds/conferences and nursing staff meetings.
  • Attends staff meetings and staff development programs to maintain and/or increase current skill level(s).
  • Charges appropriately for supplies.
  • Represents a caring, friendly and professional attitude to the patient/family or public that he/she has contact with.
  • Follows established hospital policies and infection control program.
  • Assumes responsibility for self-improvement by attendance at in-service classes, staff meetings and education related to personal growth and departmental needs.
  • Ensures proper patient care is provided in a timely and effective manner.
  • Prepares and maintains clinical records for patients, ensuring that nursing entries are accurate, meaningful and concise.
  • Seeks out opportunities to teach safe nursing practices and develop subordinates to their greatest potential.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:

· Must possess a current Alabama state license as a Licensed Practical Nurse.

· Ability to concentrate and pay close attention to detail in performing and documenting patient care and safety.

· Interpersonal skills required to instruct patients and their families, as well as, professional interaction with a variety of hospital personnel.

· Ability to perform professional nursing care, resolves complex patient care problems, and effectively manages stressful situations.

· Ability to walk and stand almost constantly, as well as, lift and position patients.

· Knowledge of hospital policies and procedures, fire, safety, disaster plans and infection control policies.

· Knowledge of patient rights.

· Appropriate skills in CPR and IV therapy.

· Ensure proper patient care is provided in a timely and effective manner.

· Prepares and maintains clinical records for patients, ensuring that nursing entries are accurate, meaningful and concise.

· Seeks out opportunities to teach safe nursing practices to develop subordinates to their greatest potential.
