Lake Martin Community Hospital is looking to hire a full-time Chemical Dependency Counselor (level 1) for our new Chemical Dependency Unit. Pay is to be determined based on experience.

JOB SUMMARY

To act as a counselor and liaison to the multidisciplinary treatment team.

PRINCIPAL DUTIES AND RESPONSIBILITIES

Completes all required forms and paperwork within regulated time period.

            Confidentiality forms

            Discharge summary.

            Progress reports

            Case Notations (Group & individual notes)

Initiates and completes Treatment Plans.

Individual Treatment Plan

Aftercare Plan

Conducts individual session with patients:  implement individual treatment plan, review problems identified on assessment, discuss aftercare alternatives, and notify of counselor availability as needed.

Conducts group therapy. (Must be knowledgeable about all therapy groups).

Attempts to establish extended treatment alternatives for follow-up care, and makes referrals as appropriate.

Ability to effectively manage manipulative and potentially volatile clients.

Knowledgeable in all areas of 12-step application to principles of daily living.

Explains the program content and structure to patients (and families if appropriate).

Reviews policies and procedures with patient including visitor privileges.

Attends education and training programs to maintain current competency.

Communicates effectively with staff, patients, families and other agencies.

Conducts self in a professional manner, representing the facility both on and off duty.

On-call to manage, assist, and/or resolve issues that may arise on the unit.

Attends at least 80% of department meetings.

Performs other duties and task as assigned by the Program Manager.

QUALIFICATIONS

EDUCATION/TRAINING

High School Diploma or Equivalent with two (2) years experience in counseling 

Bachelor’s Degree and 6 months experience in a clinical area.

Current CPR Certification

Must be able to read and write in English.

Must be proficient with computer applications, word processing and spreadsheets.

ADDITIONAL SKILLS

Must be in accordance with the standards of the applicable contracting, certifying, and/or regulatory entities including but no limited to the following: i.e. Alabama Department of Human Resources, Alabama Department of Mental Heath and Mental Retardation – Substance Abuse Services Division.

COMPLEXITY OF DUTIES AND PHYSICAL DEMANDS

Moderate to heavy physical exertion and lifting, a minimum of 25 pounds.

Extensive sitting and walking.

Normal, or corrected vision and hearing.

Ability to remain calm under stress.

Exposure to chemicals and to contagious disease.
